ABHAYRAB is a Purified Vero Cell Rabies Vaccine (PVRV) manufactured by Human Biologicals Institute (HBI), used for both post-exposure prophylaxis (PEP) after an animal bite or scratch and pre-exposure prophylaxis (PrEP) for high-risk individuals. It is a freeze-dried, inactivated vaccine reconstituted with Water for Injection before intramuscular administration, delivering a potency of at least 2.5 IU per dose as per WHO requirements. Available at VacciHub at โ‚น900 per dose with home vaccination across Delhi NCR by certified nurses following strict cold chain protocols.
๐Ÿพ WHO Rabies Exposure Categories โ€” Know Your Risk Level
Category I
Touching or feeding animals; licks on intact skin
No vaccine needed โ€” wash exposed skin with soap and water
Category II
Minor scratches or abrasions without bleeding; licks on broken skin
ABHAYRAB vaccination required immediately
Category III
Single or multiple bites/scratches with bleeding; licks on mucous membrane; bat contact
ABHAYRAB + Rabies Immunoglobulin (RIG) required immediately
โญ WHO recommends starting post-exposure prophylaxis immediately after Category II or III exposure โ€” do not wait for the animal to show symptoms

๐Ÿ‘ฅ Who Should Get ABHAYRAB?

Rabies is almost always fatal once symptoms begin, so prompt vaccination after any significant exposure is essential. ABHAYRAB is recommended for:

  • Anyone with a Category II or III bite, scratch, or exposure from a dog, cat, monkey, bat or other mammal โ€” post-exposure prophylaxis, regardless of age, pregnancy or immune status.
  • High-risk occupational groups for pre-exposure prophylaxis: veterinarians, animal handlers, wildlife and forest department staff, and laboratory workers handling rabies virus.
  • Travelers to rabies-endemic regions, including remote parts of India, with limited access to timely medical care.
  • Children, who account for a large share of dog-bite cases in India and are a priority group for pre-exposure vaccination in high-incidence areas.
  • Anyone previously fully vaccinated who is re-exposed โ€” only booster doses are needed rather than a full course again.

๐Ÿ”ฌ ABHAYRAB Composition โ€“ Per Reconstituted 0.5 mL Dose

๐Ÿ“… ABHAYRAB Vaccination Schedule

Regimen / GroupNumber of DosesTimingNotes
POST-EXPOSURE PROPHYLAXIS (PEP) โ€” AFTER A BITE OR SCRATCH
Essen Regimen (Intramuscular) 4โ€“5 Doses Day 0, 3, 7, 14 (Day 28 optional / for immunocompromised) WHO / India NRCP standard regimen; start immediately after exposure
Zagreb / 2-1-1 Regimen (Intramuscular) 4 Doses Day 0 (2 doses, one in each deltoid), Day 7 (1 dose), Day 21 (1 dose) WHO-approved alternative; completed in 3 visits instead of 4โ€“5
PRE-EXPOSURE PROPHYLAXIS (PrEP) โ€” HIGH-RISK GROUPS, BEFORE EXPOSURE
Standard PrEP Schedule 3 Doses Day 0, 7, 21 or 28 Recommended for vets, animal handlers, high-risk travelers, children in endemic areas
Booster (Re-Exposed, Previously Vaccinated) 2 Doses Day 0 and Day 3 Full course not required if a complete prior course is documented

โš ๏ธ Rabies Immunoglobulin (RIG) is required alongside the vaccine for Category III exposures (or Category II in immunocompromised individuals) and is administered separately around the wound, as advised by the treating doctor.

โš™๏ธ How Does ABHAYRAB Work?

1
Inactivated Antigen Delivered Intramuscularly 0.5 mL of reconstituted ABHAYRAB is injected into the deltoid (or anterolateral thigh in infants), delivering inactivated rabies virus glycoprotein antigens grown on Vero cells. No live virus is present, so the vaccine itself cannot cause rabies.
2
Immune Recognition and Antibody Production Antigen-presenting cells process the rabies glycoprotein and present it to T-helper cells, triggering B-cells to begin producing rabies virus neutralizing antibodies (RVNA) over the following 7โ€“14 days.
3
Passive Protection Bridges the Gap Because active immunity takes days to build up, Category III exposures also receive Rabies Immunoglobulin around the wound for immediate passive protection until vaccine-induced immunity takes over.
4
Completed Course Gives Long-Lasting Protection Finishing all doses on schedule (Essen or Zagreb regimen) generates protective antibody titers that neutralize any rabies virus at the exposure site before it can reach the central nervous system, preventing the disease.

๐Ÿšซ Precautions and Contraindications

Important Note for Post-Exposure Prophylaxis (PEP):

  • Rabies is almost always fatal once symptoms appear, so there are no absolute contraindications to post-exposure vaccination after a significant bite โ€” it must be given regardless of pregnancy, breastfeeding, age or allergy history, with reactions managed symptomatically if they occur.

Use with Caution โ€” Consult Doctor First (mainly for Pre-Exposure Prophylaxis):

  • Moderate to severe acute febrile illness โ€” defer routine PrEP dosing until recovery (not applicable to PEP, which should never be delayed)
  • History of severe allergic reaction to a previous dose of this vaccine or any of its components
  • Immunocompromised individuals (HIV, chemotherapy, chronic steroid use) โ€” may require an additional dose and antibody titer testing after the primary course

Do NOT Delay Vaccination For:

  • Mild cold, cough or low-grade fever
  • Uncertainty about the biting animal's vaccination status โ€” begin PEP immediately; it can be discontinued later only if the animal is proven healthy after the WHO-recommended 10-day observation period (dogs/cats only)
