Revac-B+ is a Hepatitis B Vaccine (rDNA) IP by Bharat Biotech International Limited. It is a recombinant DNA vaccine — HBsAg (Hepatitis B Surface Antigen) is produced in yeast cells using recombinant DNA technology, so contains no live or attenuated virus. 💡 Thiomersal — Is it safe? Thiomersal contains ethylmercury (not methyl mercury). Ethylmercury is rapidly cleared from the body and does not accumulate in tissues unlike methylmercury (found in fish). WHO, DCGI and major health authorities confirm thiomersal-containing vaccines are safe. The dose (0.05 mg) is well within established safety limits for all ages including infants.

rDNA HBsAg + Aluminium Adjuvant Injected IM
After shaking well, dose injected into anterolateral thigh (infants) or deltoid (adults). Purified recombinant HBsAg (≥20 µg) adsorbed on aluminium hydroxide forms a depot at the injection site — slowly releasing antigen for prolonged immune stimulation.

HBsAg Recognised as Foreign Antigen
Antigen-presenting cells (APCs) process the HBsAg protein and present it to naive T-helper cells via MHC class II. The aluminium adjuvant activates innate immune pathways, creating an optimal environment for adaptive immune activation.

Anti-HBs IgG Antibodies Generated
B-cells differentiate into plasma cells producing anti-HBs IgG antibodies. These antibodies bind to HBsAg on the surface of Hepatitis B virus particles, neutralising them and preventing cell entry. Anti-HBs titre ≥10 mIU/mL = seroprotection.

Long-term Memory — Lifelong Protection
Long-lived memory B and T cells are established. After the 3-dose series, 95% of healthy adults achieve seroprotection. Memory persists even as antibody titres decline — a booster dose rapidly restores protective levels. Current evidence suggests lifelong protection from a complete series in healthy individuals.

Special Situations:
Dialysis / immunocompromised: Double dose (40 µg — 2 vials) per injection, titre check after series
Non-responders: ~5% of healthy adults do not seroconvert after 3 doses; additional doses or double-dose series recommended
Pregnancy: HepB vaccine is generally safe in pregnancy — benefit vs risk is strongly favourable for HBsAg-negative pregnant women in endemic areas
Anticoagulants: Fine needle with firm post-injection pressure to minimise bleeding risk

The birth dose is the single most important Hepatitis B vaccination dose. HBV-infected mothers can transmit the virus to their newborn during delivery — and infants infected this way have a 90% chance of developing chronic HBV infection (vs only 5% in adults).
